We are creating an application that will put multiple pieces of text on to a captured video that is then saved to file.

Using GraphEdit, this seems feasible - we create 1 Overlay for each piece of text that needs to be put on the video.

Programmatically, this does not seem to be so easy. We can create a cpp mmCapture app with a single overlay easily, but how do we add more overlays to add more text fields?

You can add more than one instance of the LEAD Video Text overlay filter to the selected processors. After this, use the Capture.GetSubObjectDispatch functions with the the constant ltmmCapture_Object_SelVideoProcessor to get the first added processor. To obtain the next processor in the list, use (ltmmCapture_Object_SelVideoProcessor+1). To obtain a third processor if you have one, use (ltmmCapture_Object_SelVideoProcessor+2) and so on.
