In LEADTOOLS we give you the ability to display overlays.  You can associate one overlay with an image or multiple overlays with one image.  LEAD will automatically paint the overlay for you or you can manually paint the overlay yourself.  When we paint the overlay for you, we will scale and resize the overlay as the underlying image is scaled or resized.

These two samples shows you how to create a DICOM image containing overlays with single and multiple frames and how to read and display these DICOM files.  The projects are written in Visual Studio 6.0 C++ with MFC.  They use LEAD's API functions for displaying the Overlays and our C++ library for manipulating the DICOM Dataset.  These projects were originally written in version 15 with UNICODE enabled.

In the "SetOverlayBitmapList" project I have a Test.dic file you can use as a template.  Only certain IOD's support multi-framed overlays.  So use this template to help you get started.
