Using imaging toolkit version 16.5 Pro, have a number of areas of interest that are defined as rectangles (x,y,height,width). Simply want to build a simple overlay bitmap that shows these rectangles superimposed on the main image and moves with the main image in the image viewer. Do I need a Medical or Document licence to achieve this?

If I understand the requirements correctly, you want to display parts of one image (overlay) on top of another image (main).
One way to do that is using annotations, which is a LEADTOOLS Document Imaging feature, as follows:
1. Copy the overlay image to a temporary RasterImage and define all areas of interest as parts of the image's region (the region can consist of multiple parts).
2. Invert the region and fill the unneeded parts with a uniform color that's not part of the overlay image (to be used for transparency).
3. Create a Stamp annotation object on the viewer containing the main image and assign the temp (modified overlay) image as the Stamp object image.
4. Define the Stamp object transparent color as the color you used to fill the unneeded areas.
